We are often asked to examine plant systems or suchlike to ascertain what is causing a particular problem.

Usually this is after many attempts by others to establish the root cause and take remedial action.

Similarly we are also asked to assess the likelihood of failures and more specifically their probable causes and effects, particularly for 24/7 operations.
